⭐ NHTSA NCAP Crash Test Ratings

New Car Assessment Program

NCAP crash tests measure how well a vehicle protects occupants in controlled frontal crash, side crash, and rollover scenarios. Stars are awarded per category (5 = safest). These laboratory results are independent of owner complaints and are performed by NHTSA engineers on new production vehicles.

NCAP crash test ratings for this model are not yet loaded or this vehicle was not tested in the years analyzed. NHTSA tests a subset of new vehicles each year. Check nhtsa.gov/ratings directly for the latest results.

UNKNOWN OR OTHERMay 2026

The driver-side front door window developed a crack suddenly while the vehicle was in normal use. There was no known impact, and no object was seen or heard striking the glass. The crack originated near the edge of the window close to the side mirror and spread across the glass. There were no warning messages, alerts, or prior symptoms before the failure. The failure appeared spontaneous. This condition poses a safety concern because the integrity of the side window is compromised. The crack continued to spread, which could reduce occupant protection in the event of a side impact and may lead to sudden shattering. It also has the potential to affect driver visibility. The issue was not inspected by the dealership prior to repair. The dealership advised that they could not perform the repair in-house and directed the repair to a third-party glass facility, where the window was replaced. No definitive impact point was identified prior to replacement. Based on the fracture pattern and lack of impact evidence, the failure appears consistent with a stress-related crack rather than road debris damage. Additionally, there are multiple reports from other owners describing similar cracks occurring in the same location on this vehicle model, suggesting a possible defect trend. The original failed component is no longer available for inspection, as it was replaced during repair.

Spouse was driving Saturday [XXX] on [XXX] around [XXX], when she heard a BIG BANG out of nowhere. She discovered the next day the source was a partially exploded sunroof glass. Upon inspection there was no glass fragments in the vehicle or on the sunroof shade, fragments were only found on the outside part of the sunroof glass. She said road conditions were dry and clear and did not see anything hit the vehicle. She also stated that she did not go under any overpass during the incident. SERVICE BRAKESMar 2024

I bought this 2023 GX460 new in October 2023 from Lexus of Towson in Towson, MD. It currently has approximately 3100 miles on it. Since owning it I have noticed that when coming to a stop and sitting still with the vehicle in gear and the brake pedal engaged normally that the vehicle will start to move on it's own causing more pressure needing to be applied to keep the vehicle still. This initially caught me off guard several times. It is a repeatable problem. I can see that the engine RPM's are increasing by 100-300 by without throttle input which seems to trigger the need to apply more brake. I searched on forums and found other toyota/lexus, including GX owners complaining about this. This is dangerous for obvious reasons.

My moonroof spontaneously exploded while accelerating from a stop. We were not hit by any object and were not traveling at a high rate of speed at the time. We heard a very loud "pop" followed by a rain of glass from the moon roof. Driver sustained cuts to his hand due from the glass shards.

STEERINGJan 2024

I do have a lot problem with the GX 460 since I got the vehicle. The vehicle to the right and that the steering wheel was vibrating. We when back to the dealer the next day 4/9/23 and explained the problem to the seller that the car is leaning to the right and on the street and on the expressway between 35-45 mph the steering wheel vibrates horribly. The mechanic says he just did the inspection and everything is right with car. On 4/11/23 we went back to shop with the same problem. The mechanic says the checked the execissive road force and rotated and imbalance of tires. On 4/15/23 I went back to dealer to returned the car, because they have not solved the issue with the car. They told me they did something to vehicle but, they refused to explained why the did. On 4/24/23 I spoke to the manager from the shop and I told him if they are able to fix the car. I do not longer wanted the car. They setup an appointment with the master technician to double check what was wrong with car. The master tech told me the only thing can do is to move the steering wheel again. The car is leaning to right and steering wheels is vibrating. On 4/30/23 I spoke to Manager from the dealer and he told me that he couldn't do anything else for me. They won't be able to resolved the problem with the leaning and steering wheel. If I have any other complaints I need to call Lexus. When the vehicle is leaning to the right and pulling to the side is not consider a premium experience. The car is very problematic. Other possible cause of vibration and shaking include: bent wheel, improper alignment, worn suspension components, driveshaft problems, tire are out round and a lot money. Money that I don't have. This problem is not new for LEXUS or the dealer. There are many complaints in NHTSA and TSB in reference to LEAN to the right and Steering wheel. This is one reported L-SB-0015-21 and LEXUS haven't take any action to resolve this matter. Unfortunately, the customer is the only one who loses.

SERVICE BRAKESAug 2023

THIS VEHICLES PARKING BRAKE FAILS TO HOLD THE WEIGHT OF THE VEHICLE, UNLESS THE TRANSMISSION IS ENGAGED IN "P" (PARK). As a result, the weight of the vehicle rests on the transmission notch that is engaged with the Park position of the transmission. If the vehicle is stopped and the parking brake is engaged, the vehicle will roll in the direction of the downhill. The parking or emergency brake does NOT hold the weight of the vehicle without the transmission!

UNKNOWN OR OTHERMay 2026

My less than a year old Lexus GX550 was sitting in the driveway and the sunroof exploded. The car was not moving and it exploded outwards into a ton of pieces. The dealership claims that it is not a warrantable part. No foreign object hit the sunroof, even then it shouldn't shatter, only crack. It is my assumption that defective glass from the factory was installed into my vehicle and caused the sunroof to shatter for no apparent reason.

SERVICE BRAKESApr 2026

Car’s brake fluid leaked out of the brake lines into the master cylinder or brake booster resulting in significant loss of brake fluid to the system and a hazardous driving condition.

STRUCTUREApr 2026

The contact owns a 2025 Lexus GX 550. The contact stated that while driving at various speeds, the hood of the vehicle was fluttering up and down. The failure was more noticeable while driving 55 MPH or faster. The vehicle was taken to the dealer, who determined that the issue was normal and was known to occur with other 2025 models. The vehicle was not repaired. The manufacturer was informed of the failure, and a case was opened. The failure mileage was approximately 5.

ENGINEFeb 2026

Vehicle experiences a persistent and abrupt harsh downshift (jerk/thud) specifically during 2nd-to-1st gear transitions when rolling to a stop. This issue persists despite the dealership performing the recommended Transmission Control Module (TCM) software update at 5,000 miles. The smooth shifting returned briefly for approximately 500 miles post-update before the abruptness returned. Additionally, there is a noticeable vibration/harmonic change in the engine as the oil reaches approximately 3,000 miles of use. Concerned that this transmission behavior is a secondary symptom of internal engine drag or main bearing stress, as documented in similar V35A-FTS engine failures. This abrupt downshifting creates a safety concern regarding predictable vehicle deceleration in stop-and-go traffic. Vehicle is currently at 6,700 miles.

SUSPENSION,ENGINEJan 2026

The car has harsh knocking downshifting. In slow speeds, the has abrupt transmission changes.

TIRESOct 2025

On 10/7/25, the passenger side front tire malfunctioned - I currently have the tire. On 9/7/25, the drivers side rear tire malfunctioned - I no longer have that tire but I have included the receipt for the new tire I had to purchase. On 9/7/25, I was driving alone on the Florida Turnpike at night and in the rain, when the rear drivers side tire blew and was completely destroyed with multiple holes in the sidewall. Fortunately I was able to maintain control of the car, but I was stuck on the side of the highway until a Florida Road Ranger came to assist. The problem was reproduced on the morning of 10/7 when I found my passenger side front tire flat with a split sidewall. I am scheduled to take the car to the Lexus service department on 10/8/25 for inspection. The front passenger tire does not feel like rubber, it can be described as a mix of cardboard and plastic. On the evening of 9/7/25, when the rear drivers side tire blew, there was little warning, the low tire pressure warning light came on and I felt the flat tire in less than one minute. I purchased this new 2025 Lexus GX 550 in March of 2025 from JM Lexus in South Florida and these were the original tires. The car currently has 9,766 miles on it and used for commuting to work and running errands. [XXX] PT5A, 265 /55 R 20. STRUCTURE,VISIBILITY/WIPERSep 2025

The driver side mirror shakes around 45 to 50 miles an hour plus. The passenger side mirror at the same speeds stays true and does not vibrate. While I am driving mostly on the highway the driver side mirror shakes or bounces up and down which distorts the images behind me. It is a safety concern where I am getting dizzy while driving. I brought the car back to the dealership Lexus of Pembroke Pines, and they replaced the entire mirror assembly, but it is having the same problem again. The car is not even two weeks old. The constant vibration is a legitimate safety concern and prevents me from safely operating this vehicle.

SUSPENSION,POWER TRAIN,WHEELSSep 2025

My GX550 is vibrating at all speeds. I took my GX550 in for service on [XXX] at Butler Lexus in Union City, GA, because of the vibration in my GX. They did a wheel alignment and a force road wheel balancing but this did not solve the problem. On September 3, 2025 I took my GX550 to Discount Tires in Douglasville, GA, where they took off all the old weight including the new ones that were added by Butler Lexus. I found no improvement as I drove home. I took my GX550 back to Discount Tires, I asked them to check the tires and wheels. I was informed by one of the tech at Discount Tires that the my wheels are marginally out of round and I should let Lexus know to check the suspension. I was told by Butler Lexus service department that Discount Tires was grasping at straws. I spoke with Lexus Corporate September 19, 2025, they informed me that Butler Lexus Service department informed me that my vehicle is suppose to operate with vibrations, that could felt in the steering wheel and seats. I asked for someone from Lexus to come out and inspect the vehicle. At first I was told that Lexus doesn’t do that. Then I was told she would check into it. Also Lexus never addressed the the transmission hard shifts that I reported. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

VEHICLE SPEED CONTROLAug 2025

When accelerating from stop the vehicle has a long hesitation or lag, but allows enough movement in the vehicle to push the vehicle into an intersection. The safety concern is when pulling out across traffic to make a turn the lack of acceleration leave the drive susceptible to being hit by crossing traffic. I contact the Lexus dealership maintenance department and they indicated that it's a know problem and there isn't a recall or a solution. There is no warning light not other mechanical problem. I would not submit this concern if I didn't feel that this is a life threatening concern. There is far to much hesitation in acceleration, compounded but the fact that there is just enough acceleration to put the care in harms way with out enough to avoid a potential accident.

POWER TRAINAug 2025

On July 19th, with less than 100 miles on the odometer, the vehicle began down and upshifting so hard that it felt like a collision. Additionally, when attempting to accelerate, the transmission would hunt for gears without engaging, resulting in loss of power - creating a very dangerous situation. Lexus is unable to identify a resolution and has had the vehicle in the shop for 2+ weeks

SEATSMay 2026

The driver’s seat is making cracking, clicking, popping, and creaking noises from the lower seat frame, seat rail, or mounting area where the seat attaches to the vehicle body. The noise is most noticeable during braking, acceleration, turning, driving over uneven roads, and when shifting body weight in the seat. It feels as though the seat has movement or play, as if the seat rails, frame, or mounting bolts may not be fully secure. This is a safety concern because the seat is part of the occupant restraint system and is critical for proper seat belt positioning and airbag protection. If the seat frame, rails, or mounting hardware are loose, damaged, worn, or improperly secured, the seat could shift during a sudden stop or collision. In an accident, movement of the seat could reduce seat belt effectiveness, alter occupant positioning relative to airbags, increase the risk of impact with the steering wheel, dashboard, door, or center console, and increase the likelihood of head, neck, chest, back, pelvic, or leg injuries. Unexpected seat movement could also affect driver control immediately before or during a crash. This should not be treated as a simple interior rattle. The seat mounting bolts, rails, locking mechanisms, adjusters, frame components, and floor mounting points should be fully inspected for looseness, improper torque, wear, cracking, or structural failure. I have also found numerous similar owner complaints online from Lexus GX owners reporting clicking, cracking, rocking, or shifting driver seats that feel loose or unstable. Based on forum discussions and owner reports, there appear to be dozens of similar complaints, suggesting this may not be an isolated issue.

LANE DEPARTUREFeb 2023

i PURCHASED THE GX460 TO AQUIRE THE NEW SAFETY BSM SYSTEM. THE OWNERS MANUAL (PG366) REQIRES THE SIDE MIRROR INDICATOR LIGHT COMES ON AT 9.8 FEET BEHIND THE REAR BUMPER OF MY VEHIVLE. THE LIGHT ON THE SIDE MIRROR COMES ON WHEN THE VEHICLE PASSING ME IS AT MY REAR BUMPER MAKING IT DANGEROUS TO CHANGE LANES AND WOULD NOT ALLOW THE APPROACHING VEHICLE ENOUGH TIME TO AVOID A COLLISION. I HAVE CHECKED WITH 2 DEALERS AND BOTH SAID THERE IS NO ADJUSTMENT FOR THIS CONDITION. I FINALLY WAS ABLE TO HAVE ONE DEALER CHECK THE COMPUTER AND WAS TOLD ALL IS NORMAL. I ASKED MY DAUGHTER-IN-LAW TO WHITNESS THIS EVENT FROM THE BACK SEAT AND SHE VARIFIED MY FINDINGS.

POWER TRAIN,SERVICE BRAKES,FORWARD COLLISION AVOIDANCEApr 2026

On April 17, 2025, while driving a 2024 Lexus GX 550 Luxury Plus with a minor passenger in the vehicle, the vehicle locked up without warning as I was exiting the highway. The sudden lockup caused skid marks on the tires. The failure occurred at low speed only because I had just turned off the highway β€” had it occurred moments earlier at highway speed, the consequences could have been fatal. The failed component appears to be the drivetrain or braking/locking system. The vehicle is currently in the possession of the Lexus dealership and is available for inspection. A Lexus field inspector has been scheduled for early May 2025. Safety was put at serious risk. A child was in the vehicle. The lockup was sudden and without warning, with no ability to anticipate or respond. This incident was preceded by two prior issues with the same vehicle: (1) approximately one month prior, a transmission concern was reported to the dealer, who attributed it to abnormal driving habits and took no corrective action; (2) on April 7, 2025, brake rotors were replaced under a recall for rotors that squeak when exposed to moisture. The problem has been confirmed by the dealership, which has retained the vehicle since April 17, 2025. I am aware of at least 8 to 10 other reported incidents involving the same model vehicle with the same lockup failure. No warning lamps or messages appeared prior to the failure.

πŸ“– Understanding This Data

πŸ“‹

What is an NHTSA Complaint?

An NHTSA safety complaint is a report filed directly by a vehicle owner or driver describing a safety-related issue. Anyone can submit a complaint at NHTSA.gov. The number of complaints is a signal of how common a problem is β€” but complaints per model-year matter more than raw totals, since popular vehicles naturally receive more reports. The Lexus GX has 45 complaints on record across 10 model years.

πŸ””

What is a Safety Recall?

A safety recall is a mandatory campaign issued by NHTSA when a vehicle component poses an unreasonable risk to safety or does not comply with federal motor vehicle safety standards. Unlike complaints (which are owner-reported), recalls are formally investigated and confirmed by regulators. Manufacturers must fix recalled vehicles free of charge. The Lexus GX has 5 recall campaigns in the NHTSA database.

πŸ’₯

What Are Crash Complaints?

Crash complaints are NHTSA filings where the owner indicates a crash occurred as part of the incident. 3 of the Lexus GX's 45 complaints involved a crash. This metric is particularly important because it indicates how often defects resulted in actual collisions β€” a key safety signal beyond general reliability.

🧭

How Is the Reliability Verdict Calculated?

The reliability verdict (Average) is generated by analyzing the vehicle's total complaints, recall history, crash complaint ratio, fire complaint ratio, and top component failures relative to class averages. This verdict gives buyers a quick interpretation before they review year-level details and VIN-level history. It should be treated as one decision input, not a substitute for a VIN-specific report.

πŸ“Š

Why Do Newer Years Have Fewer Complaints?

Newer model years appear to have fewer complaints because there has been less time for owners to file them. Complaints accumulate over years of ownership. A 2020 model year vehicle will typically show fewer complaints than a 2015 model year even if it is less reliable β€” simply because fewer owners have had time to report issues. Focus on complaints per year of market presence when comparing across model years.

πŸ”

Fleet Data vs. Individual VIN

Everything on this page reflects aggregated data across all Lexus GX vehicles on the road β€” not any specific car. Two Lexus GX vehicles of the same year can have very different histories: one may have been in three accidents and have an open recall; another may be clean. A VIN check gives you the individual car's history: accidents, title brands, odometer records, service history, and real-time recall status.

πŸ”Ž

What Are NHTSA Investigations?

When NHTSA receives a pattern of related safety complaints, it opens a formal investigation. A Preliminary Evaluation (PE) is the first stage β€” NHTSA determines if a safety defect may exist. If evidence is strong, it escalates to an Engineering Analysis (EA), which can result in a mandatory recall. The Lexus GX has 0 investigations on record (0 active). Investigations listed above may have already led to the recalls shown on this page.
