I am [XXX] old, have been driving since I was 16, and until this incident, have never been in an accident. On [XXX], I was parallel parking in my friend's quiet suburban neighborhood, as we do yoga together every Sunday morning. I reversed into the space. Then I wanted to pull forward slightly to straighten out my vehicle. As I shifted into drive, I recall hearing the engine rev. My foot was on the brake but the car lurched forward at about 25 mph and rammed into the car parked in front of me, crunching his rear tail light and bumper and damaging the front of the passenger side of my vehicle. As soon as my car hit the car in front of me, it stopped. My foot was on the brake during the entire incident. Lexus is supposed to finish their 45 day investigation soon. But they did not seem concerned that I am still driving this vehicle. I am worried it could happen again and this time could be far worse. What component or system failed or malfunctioned, and is it available for inspection upon request? My Lexus 2020 UX200's brake malfunctioned and caused a crash accident into a restaurant on February 29th. It was inspected by Lexus Liability and an independent mechanic on March 13th, 2024. How was your safety or the safety of others put at risk? The accident is a horrible experience for me go through when the Lexus failed to stop without fault on my part. It put myself and people in the restaurant in great risk of serious injuries. Has the problem been reproduced or confirmed by a dealer or independent service center? The independent mechanic my husband and I hired conducted assessment and confirmed that the accident was caused by brake module malfunction. The Lexus freeze data shows Brake module voltage from ecu appears to have dropped to 8.6 volts prior to accident which caused the brake module malfunction. At the time of inspection, the voltage was 13.8. It appears that the voltage drops intermittently. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Manufacturer Lexus inspected the car on March 13th under product liability Case #240301000412. There is a police report. Insurance AAA totaled the car without inspecting the braking system. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The car was originally purchased as a brand new car in 2020 and regularly maintained by Lexus dealer. But the engine did not start one time with warning sign that the remote key can not be detected. Dealer replaced the car battery for this problem. There were times when the doors do not open for a few minutes. There were a few times that it seemed that the car did not come to a complete stop. There are many times that the car would jerk a bit and come to a sudden stop on its own when the car was driven normally and safely.

The vehicle was taken to an independent mechanic, who informed her about a transmission code and that the vehicle was stuck in the off position. The vehicle was taken to the local dealer, who informed the contact about the DTC codes that were found. The mechanic diagnosed the vehicle and located debris in the transmission and recommended that the transmission be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and offered assistance for the repair. The approximate failure mileage was 107, 000.

I am filing an urgent safety complaint regarding a defective Bridgestone tire on my 2021 Lexus UX250h, purchased new in 2021. At 32,000 miles, the [front left] tire shows abnormal tread tearing and separation, while the other tires show normal wear. This is not consistent with standard aging and indicates a potential defect and not normal wear. My attempts to Resolve it was following Bridgestone’s warranty process through Lexus and a Bridgestone dealer. Bridgestone dealer inspected the tire and blamed alignment, despite recent alignment and proper rotations done at Discount Tire Center on October 8,2025 and regularly. The wheel alignment did not show any tear. According to Lexus, Bridgestone denied the claim based solely on mileage without addressing abnormal tread separation (pls request Bridgestone response from Lexus). The tire poses serious and immediate safety risk of sudden failure, especially in rain, creating danger of a blowout, loss of control, or vehicle damage. I need to replace the tire immediately for safety. If your office requires inspection, please notify me as soon as possible so I can retain it. Tire info: Bridgestone Turanza EL450 BL 225/50R18 95V

225/50/18 Tires have less that 2000 miles on them and they are pealing at the treads. Literally whole chunks are falling off the tires…looks like someone has been picking and pulling off pieces of my tires.

This is a new vehicle. It had been serviced according to the manufacturer’s recommendations at 5000 miles, around 3 weeks prior to the date of this incident. While driving on the freeway, I got an alert that the check engine light came on and there was a β€œmalfunction in the electronic control of the engine, throttle, or automatic transmission has been detected”. Shortly after several more alerts popped up: β€œa malfunction in the hybrid system” and β€œa malfunction in the Drive Start Control System” were detected. While attempting to get to a safe location to pull over, the throttle no longer functioned. I was able to coast down an off ramp. Once stopped at the light at the off ramp, the accelerator pedal no longer worked and the vehicle was stopped blocking a lane of the off ramp. After waiting and restarting the car, I was eventually able to pull to a safe location away from the off ramp before the engine failed again.

Driving a Lexus UX300h 2025. Making a right turn from an alley onto a driveway of a house in low speed. Then the car was malfunctioning and suddenly unintended accelerated, and the front end on the driver side hit the brick structure of garage. The car is equipped with Lexus Safety System + 3.0 with Pre-Collision System (PCS). However, the system did not give any warnings to the driver of an impending collision and the Crash Imminent Braking did not apply the brakes automatically to slow or stop the car avoiding the crash.

All doors were closed and no warnings on the dash before I started driving and I made sure kids lock was engaged by checking the door lock status inside the vehicle because I had two kids sitting on the back seat with car seat installed. Sunroof exploded while driving within 24 hours of buying a new vehicle. Nothing fell on the car. We were driving in the highway and we heard an explosion, when we got out of the car to look, we saw that the sunroof exploded. I did not even know that was a thing prior to that, when I started searching after that and talking to my colleagues, seems like it was more common than I expected and is not limited to a single manufacturer. Although we were safe, glass would have fallen on us and we might have gotten into an accident because of distraction, especially when driving in highway. If this is going to be an ongoing issue, why are manufacturers being allowed to make cars with sunroof / moonroof and why are not customers informed of that while purchasing the vehicle?
