While driving in traffic, my 2023 Toyota Mirai suffered a sudden and total loss of motive power. The vehicle became completely unresponsive while in 'Drive', presenting a severe safety hazard. The dashboard displayed multiple cascading warnings, including 'FCV System Malfunction', 'Pre-Collision System Malfunction', and 'PKSB Malfunction'. The vehicle had to be towed to Dublin Toyota (Repair Order #653103). The dealership retrieved critical diagnostic code P1DC500 (FC AIR PRESSURE TOO HIGH) and verified the vehicle would not go into 'Ready' mode. The dealer opened Toyota TAS Case #TA26079027. However, the dealership performed no mechanical repairs to address the root cause of the sudden power loss. They simply cleared the safety code, test drove it, and explicitly wrote on the final invoice: 'ADVICE CUSTOMER TO DRIVE VEHICLE UNTIL PROBLEM OCCUR'. Furthermore, they refused to document my stated concerns that the vehicle is unsafe to drive. Returning a vehicle to a customer with an unresolved, life-threatening defect that causes total loss of power at speed is incredibly dangerous.
On January 26, 2026, at approximately 6:50 AM, I experienced a serious safety-related malfunction in my 2023 Toyota Mirai while entering traffic on Inglewood Avenue in Lawndale, California. I was pulling out of a parking lot into active traffic at low speed. At the moment I was entering the roadway, the vehicle suddenly lost propulsion/power without warning. Simultaneously, the vehicle’s infotainment/center display went completely black, consistent with a system reboot or electrical interruption. After a brief pause, the display rebooted and turned back on, and immediately afterward the vehicle unexpectedly surged forward. At the time of the power loss, my foot was lightly on the accelerator pedal as I was merging. When the vehicle’s systems reinitialized, the car applied power consistent with the pedal position, resulting in a sudden forward movement. The accelerator was not fully depressed, and this was not a full-throttle event, but the surge occurred without warning while the vehicle was in active traffic. The sequence of events — loss of propulsion, apparent electronic system reset, followed by unintended propulsion upon re-engagement — created a hazardous situation while entering traffic and raises serious concerns about the vehicle’s electronic control and power management systems. The vehicle’s behavior was unpredictable at a critical moment when controlled acceleration was required, which could have resulted in a collision or injury. There were no warning lights or messages prior to the event. I am reporting this incident because it appears to involve a safety defect related to loss of propulsion and unintended surge while in motion. I am scheduling a service inspection and will request a full diagnostic scan, including history and freeze-frame data from all control modules. Summary of Issue: I am the owner of a Toyota Mirai that suffered a sudden loss of power due to contaminated hydrogen fuel. The incident rendered the vehicle undrivable and raised serious safety concerns, as loss of propulsion in a fuel-cell vehicle can occur without warning while in motion. The vehicle was taken to Toyota of [XXX] for diagnosis and repair, where Toyota confirmed the presence of contaminated fuel. Toyota’s Stance: According to Toyota, the contamination issue was addressed by flushing the contaminated hydrogen from the system and refueling the vehicle. Toyota has stated that this process has restored power and that the company does not accept responsibility for any damage caused by the contamination. They have not disclosed any data on the type or level of contamination found, nor have they provided a written service report detailing inspection results or long-term corrective actions. Owner Concern: Independent research and fuel-cell engineering references indicate that contaminants such as sulfur compounds, carbon monoxide, or heavy metals can permanently degrade or “poison” the fuel-cell stack. Flushing alone cannot reverse such chemical or structural damage. As a result, there is a risk of future performance loss or total stack failure. Without transparency from Toyota regarding contaminant analysis or verification of system integrity, I cannot consider this vehicle safe or reliable to drive. Request for Resolution: I am requesting that Toyota: Provide complete written documentation of the contaminant analysis and testing performed on my vehicle. Disclose the long-term plan for ensuring the fuel-cell stack and related components have not been compromised. Offer either a full buyback/repurchase under applicable consumer protection laws or a complete fuel-cell stack replacement verified by engineering inspection. The warning that is supposed to appear every time the car is started that the car will auto shut off after one hour of idling never once appeared. As a result, my husband did not know the auto shut off feature existed on his car. He fell asleep in his car on a hot summer day and when the car auto shut off after an hour, he died of heatstroke. The lack of a warning coupled with the car auto shutting off after an hour was reproduced and confirmed by an independent service center. The car has not been inspected by anyone else. There were no warnings, messages, or other problems prior to my husband’s death. He was not informed of the auto shut off feature at the dealership at the time of purchase and the dashboard warning never once appeared to alert him such a feature existed. He thought it was safe to sleep in his running car. The auto shut off feature does not work properly as there was no warning on the dashboard and does not safeguard against people or animals being in the car at the one hour mark. Toyota needs to know this is a deadly feature and make some serious changes.
The vehicle has an advertised driving range by Toyota of 402 miles , depending on that information as true I attempted a trip about that range . During this trip and all other driving times I have never got more then 260 miles of range .

I own a 2018 Toyota Mirai in California financed through Toyota Financial. The vehicle relies on hydrogen fuel, but hydrogen stations in California are frequently unavailable or offline. Many stations are closed or out of fuel for extended periods. In addition, the price of hydrogen fuel has increased dramatically (approximately three times higher than when the vehicle was purchased). Because of these issues the vehicle often becomes unusable despite still requiring monthly loan payments. Toyota marketed the vehicle as practical to operate in California, but in reality the hydrogen infrastructure is unreliable and frequently unavailable, making the vehicle difficult or impossible to use. This situation creates a safety concern because drivers may be unable to refuel when needed.
On several occasions the Toyota Mirai would slam on the brakes on its own while at a considerable speed with no other cars around. It was very scary and concerning traveling at high speeds and the brakes being applied abruptly automatically.
The automatic braking system will lock up when there is nothing in front of the car. It will not give a warning like it supposed to. I have gotten whiplash and I’ve hurt my back several times due to this incident. It has also caused major anxiety when driving the car. It’s almost caused an accident several times. The dealership keeps telling me that nothings wrong however the issue keeps happening. Nobody’s fixing it. A specialist has been checking it out and they also were not able to find anything however the issue is still happening. There have been no warning signs or messages when the brakes lock up for no reason.
UNTIL TWO (2) DAYS AGO, I WAS THE HAPPY OWNER OF A 2017 TOYOTA MIRAI (VIN [XXX]). AT THE TIME OF MY INCIDENT, THE CAR HAD APPROXIMATELY 19,700 MILES ON THE ODOMETER. ON FRIDAY 24 AUGUST 2018 AT APPROXIMATELY 1615 (4:15 PM), I WAS DRIVING MY WIFE TO THE CLEANERS. I WAS ON ALMADEN EXPRESSWAY JUST SOUTH OF THE CAMDEN INTERSECTION IN A SOUTHERLY DIRECTION. SUDDENLY AND WITHOUT WARNING, THE CAR LOST POWER. THE CAR MONITOR SCREEN INSTRUCTED ME TO PULL OFF AS SOON AS POSSIBLE AND TO CONTACT MY CAR DEALER. I WAS INFORMED THAT I HAD AN FC FAILURE. AFTER THE CAR WAS TOWED TO THE DEALER, I WAS INFORMED BY THE DEALER THAT A SMALL ROCK HAD COMPLETELY PASSED THROUGH THE RADIATOR. THE TOYOTA MIRAI IS A HYDROGEN FUEL CELL VEHICLE. THERE IS NO INTERNAL COMBUSTION ENGINE. HYDROGEN FUEL FEEDS A FUEL CELL WHICH COMBINES THE HYDROGEN WITH FREE OXYGEN TO MAKE ELECTRICITY. THIS ELECTRICITY THEN POWERS TWO MOTORS AND/OR THE BATTERY. THE FUEL CELL REQUIRES A RADIATOR FOR COOLING. THIS RADIATOR IS THE ACHILLES HEEL OF THE MIRAI. IT IS PLACED LOW, IN FRONT, ON THE PASSENGER?S SIDE OF THE VEHICLE. THERE IS NO BUMPER TO PROTECT IT FROM COLLISIONS WITH CURBS. IN MY CASE, THERE IS NO PROTECTION FROM ROCKS AND GRAVEL HITTING THE RADIATOR AND DISABLING THE VEHICLE. THE CHILD SEAT LATCHES FOR CAR SEATS ARE NOT VISIBLE AND PRACTICALLY IMPOSSIBLE TO USE. BECAUSE OF THE DESIGN AND MATERIAL OF THE SEAT, IT TAKES A LOT OF STRENGTH AND EFFORT TO PROPERLY FIND THE LATCHES AND CONNECT THEM TO CAR SEATS. MY WIFE HAS TRIED SEVERAL TIMES TO CONNECT OUR CAR SEATS TO THE LATCH SYSTEM. IT DOESN'T MATTER WHICH CAR SEAT SHE TRIES IT WITH, SHE IS UNABLE TO CONNECT THE LATCH BECAUSE OF THE WAY THE SEAT IS DESIGNED AND THE AMOUNT OF STRENGTH YOU NEED TO HAVE BECAUSE OF THE WAY YOU HAVE TO CONNECT THE CAR SEATS. ON MOST CARS, I CAN CONNECT A CAR SEAT IN UNDER 5 MINUTES TO THE LATCHES. SOME CARS IN A MINUTE OR LESS. WITH THE MIRAI, IT TAKES ME 5-10 MINUTES TO GET THIS INSTALLED. THE LATCHES WERE MADE MANDATORY TO ASSIST PEOPLE WITH EASE OF CAR SEAT INSTALLATION BUT THE DESIGN OF THE MIRAI HAS MADE IT NEARLY IMPOSSIBLE AND IMPOSSIBLE FOR PEOPLE WHO ARE NOT STRONG ENOUGH TO MANUVER THE SEAT AND JAM THE CAR SEAT CLIPS THOUGH THE SEAT BOTTOM.

5/27/24 At 3:14 pm on I-80E exiting at 4B. I was exiting at exit 4B and was pressing on the brakes to slow down. I pressed the brakes really hard but the car kept accelerating. I tried to turn at the curve during the exit, but the car lost power steering, I could not turn the steering wheel, and ended up hitting the wall.
On August 1, 2022, I had only owned the vehicle for a few days, purchased July 28, 2022. Idiot dash lights came on. On the dash display it told me to take it vehicle back to the dealership, Tustin, Toyota, 36 Auto Circle, Tustin, CA 92680 [XXX], Service advisor claimed there was nothing wrong, lights come on when it low on fuel. One of those lights was the RED AIR BAG LIGHT. The dealership was negligent in faulty or no real inspection. I should never of been allowed to drive the vehicle out until the air bar light was dealt with. I drove away, thinking everything was okay but lights and warning signs were still flashing. [XXX] admitted to me that he was only really in charge of tires - nothing else. August 7th I was driving 20 miles per hr in Mission Viejo, residential neighborhood. The right air bag deployed, not accident and the engine was disabled by the computer in the car. I had the vehicle towed to my home and then on Monday, Aug. 8 had it towed back to the dealership, Tustin Toyota. Service writer [XXX] had a mechanic diagnosis; water from a drink had spilled on the passenger floor mat, which shorted out the wiring and circuit board, which caused the air bag to deploy. Had I been on the freeway going 65 mpg, I would have been dead. The engine stops where there is a deployment. It is ridiculous for a car manufacturer to not build the car correctly, and mount wiring, electrical and circuit boards in areas that will not encounter liquid or food issues and the vehicle owner should Not be held responsible for their design negligence. Burying instructions in small type in an owner’s manual that no way takes the time to read is negligent at best. The occupants SAFETY at all times should be the TOP PRIORITY. Purchased this vehicle on 7/30/2022 from Tustin Toyota, 36 Auto Center Drive, Tustin, CA 92782. I also purchased an extended warranty to cover all future repairs for an additional $2,995 call 'extra care'. I had some warning lights come on after only driving the vehicle for a few days. I brought spent for hours waiting for the dealership on Aug. 1, 2022. The diagnosis was there was nothing wrong and [XXX], Advisor, told me it was normal for Mirai vehicle when it gets low on fuel for warning lights to come on. Sunday, August 7, driving at 2:15 pm PST going only 20 miles per hour, on a side street in Mission Viejo, CA, the right side air bags deployed, there was no impact, no accident. The car was inoperable as the electronics sensed there was a collision so the driving mechanism was disengaged. I had the vehicle towed to dealership on 8/8/2022 for repairs. I was there 7 am. At 4 pm. I spoke with [XXX], Advisor, and he told me because water was found on the floor mat on the passenger side, it affected wiring that is underneath the floor mat and caused a short, which caused the Air Bags to Deploy. This is a manufacturers defect to mount vulnerable wiring under a floor mat. I have two warranties on the vehicle and neither warranty wants to pay for the repairs. If all the Mirai all manufactured with this wiring under a floor mat, there needs to be a Factory Recall and this wiring needs to re reouted to avoid major accidents. If I had been driving in a major freeway, I this issue could have caused major accidents and death of myself and other surrounding drivers, as it also disengages the ability to safely drive the vehicle. The only thing that should deploy air bags is a collision - not faulty mounting of wiring under a floor mat. I need immediate investigation and action on this matter as this may affect thousands of vehicles since 2014 made by Toyota. [XXX] Phone / Text. The "Parking Support Brake - PSKB" message was displayed. The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired since the extended warranty had expired, and the vehicle had a Salvage Title. The vehicle was taken to another local dealer, where it remained for further investigation. The manufacturer was made aware of the failure. The failure mileage was approximately 48,000.
The contact owns a 2022 Toyota Mirai. The contact stated that while driving approximately 60 MPH, the vehicle started losing motive power. The accelerator pedal was depressed, and the vehicle continued to decelerate. The contact stated that while driving approximately 15-20 MPH, several warning lights were illuminated, and the stability control symbol was displayed, and a yellow flashing warning light was illuminated. The yellow flashing warning light turned red, and the vehicle stalled. The instrument cluster turned black, and the steering wheel became difficult to turn. The contact attempted to restart the vehicle but was unable to shift to park(P). The contact stated that a Police Officer attempted to jumpstart the vehicle, but the vehicle failed to start, and a tow truck company was called for assistance. The vehicle was towed to the residence. The contact replaced the battery and was able to start the vehicle and the a “FCV” and the shift to park(P) message was displayed. The contact called a local dealer and was advised that the vehicle needed to be towed to the dealer for a diagnostic test. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 45,000.
1) Electrical display goes off on its own. While car in motion and driving,Distract me while I’m driving. 2) car does not start. AAA came to jumpstart, but no luck main battery under the car not responding defective battery. 3) fuel system not working properly cannot fill up the car all the way with hydrogen, it’s giving me 180 miles to maximum of 200 miles to drive ,Car slowed down on it’s own in the freeway lose power very unsafe and unreliable vehicle. 4) Blindspot, not working all the time as I am backing out it does not see other cars or pedestrian coming from left or right side or sometimes stop on it’s own with no car or pedestrian on either side. I have took this car to dealer several times and all they’re telling me they cannot duplicate the issue and I don’t believe it. this is a LEMON vehicle and very unreliable and unsafe this vehicle need to be recall. This car is now parked in my driveway I am afraid to drive this vehicle due to a safety issue .
